How Do I Fill Out and Submit My Forms?

  • We offer several easy ways to complete your paperwork.

    You can upload them directly into our dropbox below after using any of the following methods.

    Ask your physician's office to email the completed MDHHS-6200 form directly to us.

    Our team reviews submissions promptly and will contact you if any additional information is needed.

  • Computer

    Open the PDF using Chrome, Safari, or another web browser.

    Forms can be completed by clicking into the fields and typing.

    Save the completed file before submitting it through our website.

  • iOS Device

    Download the PDF and save it to the Files app.

    Open the file and use Apple's built-in Markup tools to type, sign, and save your completed form.

    Or, you can download the Adobe Acrobat Reader app (free) and use the Fill & Sign feature to complete and sign the document.

  • Android

    Download the Adobe Acrobat Reader app (free) and then download the PDF.

    Use the Fill & Sign feature to complete and sign the document.

  • By Hand

    Prefer paper? No problem.

    Print the form, complete it by hand, writing as neatly as possible.

    Scan all the pages into a single PDF before submitting.
